The Monitrice can assist in providing a continuity of care, from pre-pregnancy through post-partum. They are trained in all the basic skills that a midwife is trained, but without the certifications to actually “catch” the baby.

At the point of a woman going into labor, the Monitrice/Midwife’s Assistant attends the birth in the manner of a Doula. See the blog post: ProfessionalLaborSupport-Pt1 regarding how a Doula assists in childbirth.

A Monitrice/Midwife’s Assistant:

-Assists women with the pre-pregnancy and fertility issues they may have, along with natural birth control methods.

-Has knowledge of local resources

-Monitors the pregnancy with training in taking the blood pressure, FHT, and urine collection
They enter the labor room in a Doula capacity.

-Assist with neonatal care

-Supports breastfeeding of the baby

-May assist in post-partum care

-Knows alternative complimentary methods for pregnancy and childbirth

The childbirth educator teaches and assists women in understanding the nature of childbirth, from pre-conception through the first year of baby’s life.  The information they give assists women in having a better and safer birth experience.

The professional Childbirth Educator trained at Birth Arts International adheres to the “Midwifery Model” of care, as outlined by MANA. This is where I am training (and near completion of).

Here are some things that may be covered:

  • Nutrition – preparation to conceive, during pregnancy, and post-partum
  • Pre-natal tests: What is required and why
  • Exercise: for optimal health, and to tone muscles in preparation for birth, as well as post-partum exercises
  • Stages of labor
  • Interventions
  • C-sections and VBAC
  • Neonatal care (newborn baby care)
  • Breastfeeding

Even second-time mommies can benefit from classes.  It helps you to have a better / safer birth to review information.

Childbirth Educators can assist in labor, in a much similar way that a Doula would.  They can answer your questions and assist after the baby is born.

The issues that affect Lakota Native women during pregnancy and childbirth in regards to: Racism, Sexism, and Oppression?

Infant mortality is higher in teen births, birth weights of their babies lower, the possibility of premature birth and the birth defects more common in premature births. Premature birth is higher in this sector of the population, often due to violence during the prenatal period.
The rise in teen birth Indian country is alarming. “46% of Native American mothers are under 20 when they have their first child, compared with 25% of mothers of all other races . The average age for becoming pregnant has become lowered from mid to upper teenage years down to pre-teen groups of 9-12 year old girls!

“Boys and girls who experience sexual dating violence are more likely to initiate sex before age 11 ”. These issues would not have incurred had the elder women counseled and instructed their younger relatives on traditional women’s roles and young men counseled and instructed by elder males, as was traditionally done.

There were two ceremonies that were traditionally done for young girls, that are now rarely found in today’s Lakota Society. The pivotal ceremony for girls was the Isnati ceremony. This ceremony was done at a female’s first menses. The young girl would have had the instruction given by elder women regarding her role in society, especially as to virtuous behaviors, her place within society, pregnancy, and childbearing. women regarding her role in society, especially as to virtuous behaviors, her place within society, pregnancy, and childbearing.

Sexual Objectification of Native Women

Rape and domestic violence in Native populations have been on the rise, but within the teen population is another aspect to be considered: gangs. Gang rapes and gang violence is high among native youth, and the female population is especially vulnerable. Although there is a high risk, in one study it was reported that those who perpetrated dating violence did not use a condom deliberately despite “high risk activity such as sexual infidelity, involving “trains” and multiple sex partners ”.

As well as gang related violence and rape, young girls are also vulnerable to date rape. In 1994, “92% who had sexual intercourse reported as having been forced against their will ”. Women ages 16-24 experience the highest rates of rape and sexual assault. The violence that is inherent in this age group limits the ability of teenage girls to manage their reproductive health and also causes them to be vulnerable to sexually transmitted diseases.

1996, the rate of reported rape among Native women was 3.5 times higher than other races. This is just the reported rapes! Add to this population, those who have been subjected to another type of criminal activity: Sex Trafficking. It has occurred since the colonial era. It is only recently that the United States Government has classified Human Trafficking as a form of slavery.

Most of the Sex Trafficking occurs in areas near First Nation Reserves (Canada), Native American Reservations, and Alaskan Native communities. To understand the particular vulnerability of Native women to Sex Traffickers, you only need to look at the historical perspective. In the United States, the military that oversaw westward expansion ‘targeted native women for sexual assault, sexual mutilation, and slaughter’, as seen in numerous accounts of that time.

Compounding all the aforementioned issues is the accumulated impact of the historical experiences creating a “generational trauma” with increased levels of trauma response and stress that passed from one generation to the next, over several consecutive generations. The generational trauma is thought to be the ‘major contributor’ to the level of ‘poverty, violent victimization, depression, suicide, substance abuse, and child abuse’ in Indian country today. It is also thought to be the reason for generational prostitution and child trafficking in the Native families.

Traffickers exploit the areas in which this population has vulnerability. One method is to portray the sex trade as a quick way to become personally empowered and have financial independence. Another is to target those who are homeless or have been impacted by poverty.

Exploitation is done of those with mental illness, have substance abuse issues, FASD (Fetal Alcohol Spectrum Disorder), and those with who are Two-Spirit (transgender) are deliberately sought due to their vulnerability on the streets from violence, and are offered protection (Pierce and Koepplinger,3).

Backache

The body undergoes physical changes in pregnancy in your balance, posture, and your mobility. Your center of gravity becomes thrown off due to abdominal protrusion in the front, enlargement of your breasts, and the anterior   (frontwards) rotation of the pelvis. In order to maintain stability, a pregnant woman tends to increase the strain on the back muscles and the vertebral column.

Another cause is the shortening of the hip flexor muscle group caused by the anterior rotation of the pelvis, as well as the increased size of the muscles of the abdomen. This is not helped when a pregnant woman leads a sedentary lifestyle.

REASONS FOR BACKACHES

• Weight of the baby and the contents of the uterus
• Constipation
• Poor posture
• Standing for long lengths of time
• Urinary tract infections
• Over-working
• SCIATICA is a severe form of backache caused by the sciatic nerve being “pinched” or pressure placed on the nerve.
• The pain radiates to the legs
• Occurs due to growing baby and the womb that causes pressure on the nerve.
• The kidneys can also be affected, do the growing uterus causing pressure.

RECOMMENDATIONS

Good posture can assist sciatic nerve issues. When you improve the posture, not only will the backache be relieved but eating and breathing will improve. You find digestion is much easier as well.

Begin by keeping your head up, looking down throws your posture off. Keep your chin level. If you hold your head correctly your shoulders and back will automatically fall into place. Drop your shoulders to a position that is more natural, and avoid allowing the shoulder blades to be thrown back as it will cause a strain on your back.

Tuck your tailbone under to bring it into alignment. Pull inward the abdominal muscles and tucking the buttocks muscles…tilt your pelvis forward. This will act as a counter-balance to the tendency of arching the back.

• Wear flat comfortable shoes. Keep your knees relaxed, not locked
• Sit up straight in chairs and when you drive

Do stretching exercises, and / or exercise by walking briskly for 30 minutes a day, leg lifts and lunges, swimming, or Yoga. Avoid too many weight-bearing exercises . Doing exercise relieves muscle tension. It also Relieves emotional tension. Another thing you can do is to rock your hips, or make love passionately (no joke), the latter relieves pelvic congestion.

Relieve constipation:

• Constipation is directly connected to lower back pain and pelvic discomfort
• To keep from having constipation, drink more water and eat whole grains.
• Increase Calcium and magnesium (see: minerals hand-out).

Elevate your legs, preferably for 20 minutes a day (up on a chair or lying down with two pillows under them)

DIETARY RECOMMENDATIONS

Keep cold or raw foods to a minimum (these increase kidney strain). Eat grapes, pears, and apples as these are the least “watery” of the fruits and are less strain on the kidneys.

Minimalize fruit juices, and caffeine. (coffee, black tea, chocolate, cocoa, and soda). They act like adrenaline in the system due the chemicals they contain. Adrenaline aggravates the kidneys.

HERBAL RECOMMENDATIONS

If you are tired, feel overworked, or stressed you may be experiencing adrenal gland deficiency. The best herb to nourish the adrenals is nettle leaf, in a strong infusion (steeping for at least ½ hour). Nettle Leaf is one of the herbs found in your “Pregnancy Tea”.

Take St. John’s Wort and skullcap in a tincture form. About 20 to 30 drops in water or juice a couple of times a day. You can also apply the combination of Arnica and St. John’s Wort oils externally to relieve tension and promote the healing of the muscles of the back. It’s also helpful to use a warm water bottle on the area that has been treated with Arnica and / or St. John’s Wort.

The biological age for a “mature” woman is 18 years of age.

A woman of this age and older physically is able to handle the demands of a growing baby, along with delivery.

Realistically though, the best success is between the ages of 25-34 years old.

The uterus of a teenager is often not structurally nor functionally fully developed. The teenage uterus would not respond to the hormones produced naturally by the ovaries in the same way as a fully mature uterus.


The risks of a teen pregnancy are:

• Preterm birth
• Low birth-rate babies


The complications are:

• Infections
• Reduced intelligence
• Many babies born to teen-agers do not “catch up” as they mature.
o Requiring special attention
o Special schooling
o Special healthcare

Many of the problems of health care in Teen Pregnancies are due to insufficient nutrient intake.

The need for vitamins and minerals are at an all-time high for the teenage girl, add that to the demands of a growing fetus and the struggle between the baby’s needs and the teen mother’s need begin.


The struggle for nutrients can:

• Increase the uterine blood flow in the 3rd trimester
• Reduce availability of nutrients in the mother’s blood
• Limit the transmission of nutrients from the mother to the baby, which interferes with the baby’s development, and birth weight.

To compound matters, teenagers tend to fill-up on less nutrient-rich foods such as:

French fries, soda pop, chips, candy, etc.

The average teenager eats food too high in fats and salt, and too low in fiber and nutrients. Teens that eat foods that are processed and highly sweetened are at high risk for low birth-weight babies who are an elevated risk for disease and death.

Weight Gain in Pregnancy

Having a healthy baby with a normal birth rate
is directly linked to pre-pregnancy weight.

 

Even if you are 20 or more pounds over-weight you would still need to gain weight during your pregnancy.

Many women know little about the importance of weight gain in pregnancy, or are given too low of a weight amount. Not gaining enough weight puts your baby at risk for infant low birth weight or fetal death.

Women who gain less than 20 pounds during pregnancy are at the highest risk. The best fetal outcome is when a woman gains twenty-six to thirty-five pounds during their pregnancy.

A woman who is overweight at conception, may need to go up to 40 pounds in weight gain. Her calorie intake was already higher when she conceived. She needs to consider the needs of the unborn fetus inside of her.


DO NOT DIET DURING YOUR PREGNANCY!

 

Under-eating during the last portion of the pregnancy puts the fetus at risk for underdevelopment of the brain, and abnormal growth.

• Don’t worry about your weight gain.
• Eat when your appetite tells you to eat.
• But…when you eat, eat with good nutrition in mind. No “empty calories”, …ever.
• Don’t restrict salt intake
• Be careful with drugs, especially diuretics.

During pregnancy the blood volume of the circulatory system increases more than 40 percent in order to take care of the needs of the mother and baby. The expansion of blood is determined and is maintained by whether or not there is adequate salt intake. This includes salt that is found in the foods you eat, as well as that which is used to flavor the food.

Be sure to refer to the “Baby Wise Diet” blog post or hand-out when choosing foods. Eat fresh whole foods as often as possible. Try to limit the “sweets” from your diet such as cakes, pies, cookies, and candy. Also limit fast foods and fried foods.


Concentrate on:

Fruits, vegetables, whole grain cereals and breads, fish (see nutrition hand-outs for cautions), poultry and lean meats.

Dairy products are also important, if tolerated, but remember calcium and other nutrients can be found in dark leafy vegetables as well.

Yogurt can be and is a good source of protein and calcium, but also is good for the intestinal tract. It is better tolerated than whole milk because of the fermentation processed used to culture the yogurt. With the over-use of antibiotics, our intestinal flora cannot support the production of essential vitamins normally found within the intestinal tract.

  • To INFORM women about all aspects of pregnancy and labor, in order to take the fear out of childbirth.
  • TRAIN women in nature’s process of birth, breastfeeding, and good nutrition.
  • Teach RELAXATION techniques, to reduce discomfort during childbirth.
  • Help women with techniques to make childbirth EASIER.
  • The result should be more SATISFACTION.

Women have helped and supported other women in childbirth since the dawn of time. In our modern times, this practice has diminished. Today we have been subjected to the medical/scientific process that is not focused on the natural way. It is geared more towards heroic measures, mostly based on erroneous ideas or for the time-structure of the doctor, rather than the timing of nature in a natural childbirth.

“The house for giving birth (having babies)”. It is my goal to develop a free-standing birthing center on the Rosebud Reservation. I also wish to have a converted bus to travel around to the various communities on the reservation and bring birth support to the individual women on the reservation.

Childbirth Education

This is a source site for information about infertility/fertility issues, preparation for pregnancy,  healthy pregnancies, a safe childbirth, and Post-Partum care, and breastfeeding. The Rosebud Tribal Education has its own services for educating woman on the reservation for Parenting Classes. These two are separate and yet linked groups of topics.

Here you will also find support for those who wish to assist women and teens in pregnancy, links to information about childbirth, and a portal for online short-course Childbirth Education, for those who cannot afford/have no insurance for a class or have no transportation.  There will also be a page of links for information online.

Childbirth is a normal process.

Long before doctors took charge, women were helping women to give birth.

These women were called midwives.
